Social Care Assessments

We provide comprehensive Social Care Assessments to help individuals, families and carers understand their care and support needs. This includes:

  • Identifying a person's strengths, difficulties and circumstances.
  • Discussing how their wellbeing is affected and can be promoted.
  • Exploring the outcomes that an individual wants to achieve.
  • Determining how this summary of needs meets the Care and Support Regulations eligibility. 
  • Recommending how an individual, and their carers, support needs can be most appropriately met.
  • Specifying the type of provisions required.
