Social Care Assessments

I provide comprehensive Social Care Assessments to help individuals, families and carers understand their care and support needs. This includes:

  • Identifying a person's strengths, difficulties and circumstances.
  • Discussing how their wellbeing is affected and can be promoted.
  • Exploring the outcomes that an individual wants to achieve.
  • Determining how this summary of needs meets the Care and Support Regulations eligibility. 
  • Recommending how an individual, and their carers, support needs can be most appropriately met.
  • Specifying the type of provisions required.

Imara's assessments are conducted in accordance with the Care Act 2014 to ensure they are legally sound and meet the highest quality standards. I aim to provide a positive experience, that is sensitively planned and undertaken to put the individual concerned and their families at ease.
