We provide comprehensive assessments and recommend plans to assist young people and their families in preparing for adulthood. This is sometimes referred to as 'transition planning' and is an essential step so that young people are proactively supported to navigate the changes ahead and ultimately achieve their goals. Very often this isn't just about practically planning but navigating the legal frameworks and taking the necessary steps so life plans can progress in the manner envisaged. This might involve:
- Assessing what support a person may need by exploring their strengths, difficulties, family and community support networks.
- Identifying different housing and support options, including opportunities and risks.
- Exploring a person's goals and aspirations, and recommending a plan so that a person has an active, fulfilling and purposeful day.
- Exploring if a young person needs support with decision making and making the necessary arrangements to determine this and seek any court orders in good time, should this be necessary. Key decision points tend to be related to accommodation, signing a tenancy and managing money.
- Exploring if the arrangements in adulthood amount to a 'deprivation of liberty' and ensuring the necessary considerations have been made and actioned as appropriate so that a person's rights are safeguarded.
Our assessments and recommendations are grounded in the relevant frameworks such as the Care Act 2014, Social Services and Wellbeing 2014 Act and Mental Capacity Act 2014.
